Community Outreach Referral Coordinator
Central NY
Conifer Park, an employee owned company and premier Substance Abuse treatment provider in NY State is seeking an entrepreneurial driven and results oriented individual to join our field based Community Outreach Referral Coordinator Team for our Central NY territory.
The Community Outreach Referral Coordinator will outreach to various referral sources that include but are not limited to hospitals, outpatient programs, private practitioners, physicians, community based organizations, seeking patient referrals into Conifer Park’s inpatient and outpatient system.
Additionally the Community Outreach Referral Coordinator will develop and host educational in-services to best position Conifer Park in the region as the subject matter expert on Substance Abuse treatment and seek business opportunities with potential partners.
Requirements:The ideal candidate can hold a Bachelor’s degree in a clinical discipline and/or certification as a CRPA, with concomitant life experiences. Candidates should possess strong engagement skills, comfortability to work independently in the field; strength with customer service and strong organization and communication skills.
