COVID Project Community Health Worker

Supervisor: COVID Project Coordinator

POSITION SUMMARY:

The Community Health Worker (CHW) is an individual who utilizes their experience and knowledge of the language and culture of the population he or she serves to facilitate bridging between patients and resources in the community. Community Health Workers provide health education and information and work with patients to effectively identify barriers to care and facilitate connections in the community to meet patients’ needs. The Community Health Worker should be comfortable with and have the ability to work effectively in a busy environment and can demonstrate flexibility to meet the changing needs of the patients and the Health Center.

The COVID Projects CHW, is an individual who combines both his/her knowledge and connection to the community to further BNHC’s COVID Response. Reporting to the COVID Projects Coordinator, this individual works full time, on-site, mainly at the testing sites, to support BNHC’s testing efforts particularly within the populations in Brockton that have been most negatively impacted by COVID-19.

PRINCIPAL DUTIES AND RESPONSIBILITIES:
  • Provide health education about COVID-19 (testing, public health safety measures, contact tracing and vaccination and treatment information).
  • Complete COVID Project related activities as assigned, in order for the projects to successfully reach their objectives and goals
  • Provide culturally appropriate health education, information and, as needed, interpreter services for patients participating in BNHC COVID Care (particularly testing and vaccinations)
  • Communicate effectively and positively with patients, community partners, and with an inter-disciplinary team of professionals at the health center.
  • Engage in outreach efforts as determined by the Health Center to “meet patients where they are”
  • Provide direct services, including informal counseling and social support for patients as applicable in COVID related activities
  • Advocate for patients’ needs and support care coordination to meet those needs at the time of testing or vaccination
  • Complete all required documentation in a thorough and timely manner as required by Supervisor and per BNHC policy.
  • Demonstrate HIPPA compliance and maintain patient confidentiality at all times.
  • Attend all required meetings as determined by Supervisor

QUALITY ASSURANCE
  • Participate in regular supervision facilitated by a Supervisor.
  • Actively Participate in necessary meetings associated with COVID projects
  • Maintains excellent attendance
  • Complete all assigned myNetLearning Trainings
QUALIFICATIONS:

High School Diploma or Equivalent.
Bilingual Required. Multilingual Preferred
Medical interpreting certificate for CVC, Portuguese, Spanish, Haitian Creole or French preferred

MINIMAL KNOWLEDGE BASE:
  • Ability to read and write in English
  • Ability to use technology effectively
  • Understanding of the Brockton Community, available resources and needs of the population served
WORKING CONDITIONS:
  • BNHC is a busy community health center located in Brockton’s Downtown Business District.
  • One late day per week may be required to accommodate patients attending evening appointments.
  • Occasional travel as required per job responsibilities
PHYSICAL REQUIREMENTS:
  • Physical demands requiring lifting of small pieces of equipment and/or boxes not to exceed 20 pounds.
  • Mostly sedentary and interoffice walking required.
  • Driving and walking in the community as required.
  • Visual acuity sufficient for frequent reading.
  • Hearing acuity sufficient for holding conversations with or without audio devices.
In order to prevent a direct threat to the safety and health of our patients and staff, effective August 1, 2021, new hires are required to have had at least the first dose of a COVID vaccine prior to their start, with the exception of those who have documented medical or religious exemptions. New hires who are not yet vaccinated can arrange to have the vaccine on site.
