The purpose of the Health Office Nurse is to provide nursing support and supervision to the residents in Residential Living, including operation of the clinic and home visits.
NATURE AND SCOPE
This position reports directly to the Executive Director and is both nursing and clerical in nature. This position respectfully interacts with all residents, family members, visitors and co-workers, and maintains a positive customer service relationship practicing honesty and integrity in all aspects of job performance. In performance of duties, the Clinic Nurse is entrusted with the following responsibilities:
Complete Life Connect plans for all residents. Maintain an individual file on each resident, including an annual visit.
Educate residents in regard to emergency procedures such as calling 911 appropriately.
Assist residents in locating and procuring home care services and equipment as is appropriate for their needs.
Consult with residents in their apartment who are complaining of illness or in need of additional services.
Advises family members, administration, physician and other appropriate parties of significant changes in health of residents and/or anticipated changes in level of care. Assists residents in making referrals to our campus therapy team as appropriate.
Schedule resident care conferences to share concerns and recommend options for care to families in coordination with the interdisciplinary team.
Maintain adequate supplies and equipment in the Clinic.
Respond to emergency needs of residents by calling 911, physicians, ambulances, etc.
Participates in planning for transfers/discharges to and from Residential Living and the Healthcare Facility or hospital.
Prepare residents for admission to the Health Facility or hospital.
Participates on the Resident Assessment Committee.

Clinic responsibilities include: blood pressure clinic, vaccine clinics, COVID testing of residents, temperature checks if needed, etc.
Keep Vile of Life information up to date and on back of each resident door.
Input information/update Vision as needed.
Attend monthly Townhall meetings to update residents; attend weekly Life Connect/Transitions meetings and attend Leadership meetings.
Arrange tray services for residents who are ill.
QUALIFICATIONS
This position requires a Licensed Practical Nurse or Registered Nurse in the state of Michigan. The individual must possess a current, active license, which must be maintained at all times while working in this position.
Two (2) years of experience working in nursing with senior adults.
The position requires good general nursing skills. The capacity to maintain good professional relationships with physicians and relate well to patients and families is necessary. The ability to understand and show compassion to senior adults is essential. Good observational skills are required.
Qualities such as genuine compassion for the elderly and disabled, project a friendly personality, understand customer service and team building concepts, ability to deal with residents in a pleasant and professional manner is required.
Level of ability needed to work independently or with direct supervision, ability to follow through on assigned tasks, demonstrate initiative, etc.
Ability to prioritize tasks.
