Case Manager: Salary-low, mid to High 30's Negotiable depending on education and experience. Full-time 40 hours per week. 8-5- Monday-Friday. The basic functions of the position are linked through virtual home visitation services. Program Region: Scott, Mississippi, New Madrid, Pemiscot, and Dunklin counties.

Provides virtual home visitation in office settings and provides intake, referral, and educational services for clients enrolled in the Missouri Bootheel Healthy Start case management program. These services are provided during the pre-natal and post-natal care period to pregnant women, newborns, and infants up to 18 months. Manage assigned caseload, maintains HIPPA compliance, and have strong proficiency with computers and database entry preferred. Demonstrate ability to work in a team environment and independently with supervision. Demonstrate good verbal and written communication, interpersonal and listening skills, and culturally sensitive to the population served.

Qualifications: Experience in case management, maternal and child health, and infant or mental health in a non-profit setting is preferred but not required. Associate degree or some college preferred in a related field but not required. Experience will be considered in place of education.
