*Transitional Housing Program (THS) offers both a Sign-On Incentive AND a Retention Incentive range between $750-$1500 based on annual salary/pay.*

Schedule: Mon – Thurs, 9am – 6pm, flex every other Fri 9am – 5pm
Pay Rate: $21.00

JOB SUMMARY:
Under the general direction of the Supportive Housing Program Director, perform intake coordinator responsibilities and provide focused services to Supportive Housing aftercare clients. Duties and responsibilities include but are not limited to: Identifying clients in need of services through the Coordinated Entry System (CES), completing the intake process internally and externally, and engaging with alumni in aftercare services.
ESSENTIAL JOB DUTIES:
1. Will act as primary liaison with all facets of the criminal justice system, child welfare system, and agencies in accordance with program objectives, pertinent rules, regulations, and laws.
2. Create and document processes, maintain notes, and complete records, forms, and reports in a timely and efficient manner.
3. Develop an effective rapport with clients and facilitate orientation programs which include but are not limited to: explaining rules, policies, and regulations to clients, and determining program eligibility.
4. Conduct preliminary interviews with clients to determine client’s appropriateness for the program to include: reviewing documents from Probation and/or Department of Children and Family Services (DCFS), Coordinated Entry System (CES), Los Angeles Homeless Services Authority (LAHSA), and other medical documentation to ensure proper care and process of the client and their records; prepare files for periodic review and make recommendations.
5. Collaborate with the Family Advocates during the discharge planning process to ensure continuity of care from Transitional Housing to Permanent Housing.
6. Coordinate and schedule interviews and assessments with clients.
7. Provide referral information for other housing and shelter options to professionals from outside organizations and individuals who do not qualify for the program.
8. Establish and maintain contact with community agencies, resources, and professionals necessary for proper service provisions to clients.
9. Maintain a database and collect all intake process data. Conduct and provide monthly intake reports to Supportive Housing Program Director.
10. Collaborate with referral agencies, including DCFS/Probation, in procuring appropriate documentation.
11. Coordinate alumni program by engaging all participating alumni in follow-up aftercare services.
12. Develop and facilitate events for THP residents and alumni.
SECONDARY JOB DUTIES:

  • Provide crisis intervention to residents and develop safety plans to assist residents with making safe choices.
  • Maintain a healthy, therapeutic, and safe environment for all residents.
  • Actively participate in mandatory training, in-services, and meetings.
  • Assist with the operations of the lobby when necessary.
  • Assist during emergencies. Direct emergency response personnel, residents, and staff. Assist with evacuating the building if necessary.
  • Participate in the Continuous Quality Improvement (PQI) of the agency.
  • Communicate through fax, telephone, and e-mail with other service providers when appropriate.
  • Maintain confidentiality as required by St. Anne’s, Licensing, HIPAA, and other regulatory agencies.
  • Other duties as assigned

QUALIFICATIONS:

  • Bachelor’s Degree in a Behavioral Science and a minimum two years of full-time experience working with transition-aged youth, or a Master’s Degree in a Behavioral Science or Human Service.
  • Bilingual in Spanish/English is preferred.
  • Good oral, written and organizational skills.
  • Must possess and maintain a valid California Driver’s License and a driving record acceptable to the agency’s insurance carrier.
  • Physical ability to perform Pro-ACT, administer first aid/CPR and operate an Agency motor vehicle.
  • Ability to independently use a computer and the ability to learn necessary computer programs.
  • Ability to receive and implement constructive feedback.

2. The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
3. Employee may occasionally be required to drive a motor vehicle.
