Community Health Network is seeking a full-time Behavioral Health Medication Coordinator this role will be to primarily support behavioral health integration medication refill request messages. The ideal candidate will communicate with patient, screen for negative side effects, assess medication compliance, and transmit refills to the patient’s preferred pharmacy.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Performs medication refills for behavioral health patients and behavioral health integration according to CHN standards of care for psychiatry and adult medicine medication refills.
  • Patients will be screened for side effects, medication supply and taking prescriptions as prescribed.
  • The medication coordinator will communicate with providers regarding side effects or refill request clarifications as needed.
  • Ensures accurate and appropriate response to messages and requests from multiple sources.
  • Assist patients in enrolling and re-enrolling with Prescription Assistance Programs.
  • Assist in completion of prior authorizations requests and communication of approvals or details to care team as appropriate for medication access
  • Be aware of new additions or changes to medications/programs that could be useful to partner clinics and patients.
  • Ensure best practices are applied to all procedures: adhering to HIPPA regulations and maintaining timely and accurate electronic records.
  • Assist in resolving patient complaints or identified issues in a timely manner
  • Contribute to continuous process improvements.
  • Manage database and track all information for reporting and analytics; maintain reports, spreadsheets, log sheets and forms as necessary.
  • Support delegation agent for psychiatry.
  • Build and manage relationships with patients, managers, providers, pharmacy staff, clinic staff and patient support to best serve patients.
  • This position is a designated agent for electronic prescription drug orders including controlled substances.
  • The behavioral health medication coordinator promotes improves utilization of CHN pharmacy for medication adherence.

QUALIFICATIONS:

  • High school diploma or GED
  • Bilingual preferred: Spanish speaking
  • Medical Assistant (MA) with at least 2 years’ experience
  • Knowledge of all confidentiality requirements regarding patients and strict maintenance of proper confidentiality on all such information.
  • DEA training, competency, and privileging authorized by the Prescriptive and Delegating Physician to DEA registration number to send prescribed controlled substances on behalf of the Prescriptive and Delegating Physician.
  • Ability to deal professionally, courteously, and efficiently with the public and all levels of the organization.
  • Effective language, listening and verbal presentation skills to communicate with staff, patients, families, and caregivers.
  • Ability to read and understand complex information and reference materials to collect relevant information.
  • Ability to utilize technology (phone, internet, EMR) to conduct patient visits and complete day to day tasks.

SALARY INFORMATION:

Factors used in determining the salary offered include the candidate's qualifications as well as the pay rates of other employees in similar positions.

ADDITIONAL JOB-RELATED INFORMATION:

  • Monday - Friday, 8:00PM - 5:00PM
  • This position may require a rotating weekend as needed
