Independence Plus is rapidly expanding in all of our service areas and is now hiring a full-time Nursing Manager (also called Clinical Manager; must be a Registered Nurse) to work out of our Boerne, TX (Kendall County) office. This position coordinates and supervises Home Nursing and Home Health Services in accordance with the Physician Plan of Care, with responsibility for compliance with rules and regulations as required by Medicare, the State, and Independence Plus. Reports directly to the Director of Clinical Services and has flexibility to provide clinical education and patient care as needed.

JOB FUNCTIONS AND RESPONSIBILITIES:
  • Reviews patient clinical diagnoses, medications, procedures, and clinical course and develops therapeutic patient goals
  • Provides the Starts of Care (SOC), Resumptions of Care, and Recertifications of Care (ROC); completes the 485 and OASIS documentation
  • Directly supervises clinical staff via phone and home visits as required
  • Performs skilled nursing visits as needed and coordinates adjunct therapies
  • Complies with Company policies and procedures, State and Federal Regulations, Medicare, and other payer source requirements
  • Possesses a valid state driver’s license and maintains a good driving record
  • Other duties as assigned

QUALIFICATIONS:
  • Current Registered Nurse (RN) license
  • Minimum one year of home health clinical experience preferred
  • Current CPR certification (AHA Healthcare Provider)
  • Supervisory experience preferred
