Outreach services include educating the homeless community about existing social services, helping register cars and RV’s, adding individuals to the key tracking system that impact homeless policy and ultimately getting them on the official path to permanent housing.

Job Description/Purpose: This position involves coordinating the delivery of front-line life-services; assessment of basic stability needs of those experiencing or recovering from homelessness in Sonoma County to help stabilize unsheltered people with a goal of improving their conditions and getting them into permanent housing.

Required Minimum Qualifications: Some education or experience in social work, the retail industry, or the Sonoma Continuum of Care system is preferred. Basic first aid skills. Some computer experience that contributes to an ability to use HMIS for data entry. Driver’s license, valid in CA. Must be vaccinated against COVID, including a booster. Bilingual Spanish/English is a plus.

Essential Job Functions:

· Visit unsheltered persons in a variety of and often changing locations, i.e, in homeless camps, in parks, and along trails, to provide shelter information and help individuals in a variety of ways.

· Work as part of a closely knit team to achieve goals.

· Communicate with other service providers to share information and make plans.

· Obey HIPAA laws, and teach volunteers and clients about their information rights and responsibilities.

· Food coordination, preparation, pick up, transport, and delivery,

· Administrative support such as notary coordination, ID application, Coordinated Entry, SOAR/SSI signup or coordination, Medi-Cal, Medicare, Cal-Fresh and other program signups. Make referrals to other agencies and programs as needed.

· Drive vans, (provide transportation of individuals or groups to services and doctor's appointments.)

· Fill out required reporting forms daily; enter data into a database each day.

Working Conditions and Physical Requirements:

-The Outreach Worker will work in various Sonoma County locations, independently and within small teams. Must have the ability to lift 20 pounds; and the ability to get into and out of the SAVS van and walk short distances over rough terrain.

Knowledge, Skills, and Abilities: All staff require: Ability to communicate effectively; work collaboratively with co-workers; understanding of local homeless programs; a medium or high level of personal organization; maintenance of excellent records of activities and status as required; a high level of personal reliability, in an environment where inconsistency is the norm; a relatively high level of autonomy, since most work will be done without direct supervision; a respectful, professional approach to client care; great patience with challenging clients; an ability to re-prioritize quickly and easily depending on the day's needs; all staff must develop a broader base of skills than they have, so trainability is essential, i.e., maintaining a healthy attitude toward guidance and correction; ability to follow through on instruction without undue repetition or continuous guidance.

Pay: $19.00 - $21.00 per hour
