Job Summary
- Provides patient care consistent with the Mission and Vision of Avera.
- The Population Health Pharmacist will be responsible for carrying out medication-related strategies designed to optimize patient care, improve outcomes, and reduce total cost of care in a value-based care population. The pharmacist will provide a high level of patient care to value-based care patients; close medication-related care gaps; optimize medication therapy through completion of comprehensive and targeted medication management; work collaboratively with the value-based multidisciplinary care team; support the pharmacy's pharmacoeconomic efforts; and providing drug information and education to patients, physicians, nurses, and other healthcare professionals. Additionally, the pharmacist will be responsible for assisting in the design and implementation of clinical projects and pharmacy services.
Education and/or Experience
- Doctorate of Pharmacy Degree preferred.
- Completion of ASHP Accredited PGY1 (and PGY2 for some areas) Pharmacy Residency preferred.
Licensure/Certification/Registration
- Registered Pharmacist in the state of South Dakota required within 30 days of hire.
Work Schedule
- Monday-Friday, 8:00am-5:00pm; 80 hours every 2 weeks
