We are seeking a Contract Occupational and Physical Therapist for our following Suffolk, VA facilities for our Residential program:
Responsibilities and Duties for Occupational and Physical Therapist:

• Recommendation of Home Health exercises
• Document progress and activities, tasks performed, and response to
• provide an initial assessment of all current equipment in use by the agency, as well as aid with locating resources for the purchase of new pliable equipment.
• Report any consumer condition changes to the RN and Program Supervisor.
• provide training for new exercise/positions recommended for consumers, and on new adaptive equipment.
• Comply with agency policies and procedures, and adhere to all relevant local, state, and federal legal and regulatory requirements.
• Complete quarterly and annual reports as required.
• Participating in interdisciplinary team as in person orby written report.

Qualifications and Skills for Occupational and Physical Therapist:

• Licensure as a Occupational/Physical Therapist
• Must have liability coverage
• 1 year of PT/OT experience
• Valid driver’s license with acceptable DMV record
• Experience with those with mental health concerns or in a behavioral healthcare environment
• Proficient using Electronic medical records
• Basic knowledge of Medicaid/Medicare Regulations preferred

Work Schedule:

• Flexible work hours based upon the contractor’s availability.
