Job Summary

Basic Function: The Registered Dietitian Nutritionist (RDN) is an integral member of the adult kidney pancreas transplant team at University of Michigan Health (U-M Health). The RDN has their own provider schedule to see patients referred by the medical provider team or per transplant standards. Patients and RDNs meet via 1-on-1 appointments, and/or group virtual visits. The RDN serves as the nutrition expert who cares for people during evaluation for transplant and overall pre-transplant care. All kidney transplant candidates will be assessed by the transplant RDN during evaluation process. The RDN’s work with transplant patients often includes work to optimize self-management of Diabetes and weight before and after transplant . This RDN will also help care for acute transplant patients within the first 12 weeks of transplant. This role includes participation in weekly transplant selection meetings, as well as other department related meetings such as Transplant quality meetings. This RDN may often work with dialysis units as necessary for care coordination.

The RDN provides service to adult patients over the age of 18 years.

Organizational relationships Supervision received from Manager of Ambulatory Care Nutrition Services. Leadership oversight also received from Transplant and physician leadership. Liaison relationship with multidisciplinary health care team for patient care populations of responsibility. Peer relationships with RDNs in work group, including other transplant RDNs well-established on team. RDN may precept dietetic interns.

Responsibilities*

Patient Care Conduct an ongoing comprehensive nutrition assessment for patients identified at nutritional risk and/or for patients identified to be seen as per ambulatory clinic standards. Incorporate evidence-based standards when interpreting patient related data. Organize patient health and disease information to identify nutrition-related problems. Evaluate need for specialized nutrition support and make appropriate recommendations accordingly. Develop and implement an individualized plan for nutrition intervention. Perform nutrition diagnosis after identifying the problem and clarifying the cause of the problem. Ground interventions through review of quality research and/or use of medical nutrition therapy evidenced-based practice. Appropriately document individualized nutrition treatment plan, including follow-up documentation of patient outcomes and revisions to plan in the electronic medical record. This communication is performed in a timely manner that is consistent with unit policy and procedures. Recognize the varied needs of age-specific populations, as well as cultural, religious, and ethnic concerns. Reassess the nutrition care process and implement changes as indicated. Communicate regularly with the medical team through interdisciplinary ambulatory care clinic coverage. Abide by U-M Health policy for the proper use of patients’ protected health information. Abide by U-M Health policy for the proper use of infection prevention precautions in patient care areas.

Business Functions Provide service to the ambulatory adult transplant clinics for identified patient populations, including virtual care via 1:1 appointments, group visits, and 1:1 telephone support. Has routine communication with patients as needed for frequent diet related questions on very complicated diet regimens or when other nutrition issues arise. Utilize charge capture by RDN for RDN services. Provide quality service based on customer expectations and needs. Incorporate U-M Health core values of respect, compassion, trust, integrity, efficiency and leadership into all work efforts. Maintain productivity standards and practice effective time management. Assist in achieving compliance with state and federal regulatory and accrediting agencies. Participate in interdisciplinary meetings, task forces, and projects as appropriate. Contribute to quality improvement efforts. Collect outcome data to help improve practice as requested. Exemplify and model equitable behavior that encourages belonging and respect in an inclusive, diverse work environment. Other duties as assigned.
Required Qualifications*

Required Qualifications
Bachelor’s degree granted by an accredited college or university or equivalent combination of education and experience is required.
Completion of a didactic program in dietetics and supervised practice program approved by ACEND (Accreditation Council for Education in Nutrition and Dietetics) of the Academy of Nutrition and Dietetics.
Registered Dietitian Nutritionist with active registration by the Commission on Dietetic Registration of the Academy of Nutrition and Dietetics. Requires primary source verification.

KNOWLEDGE AND SKILLS REQUIREMENTS Able to work with a team to accomplish organizational goals. Communicate in a clear, concise, and diplomatic manner both verbally and in writing. Implement critical thinking to solve problems. Use technology such as word processing, spreadsheet, and e-mail.
Desired Qualifications*

Master’s degree in Dietetics, Nutritional Science, Public Health, Health Behavior/Health Education is preferred.
Documented experience working with transplant and/or renal nutrition related care.
Professional member of the Academy of Nutrition and Dietetics, National Kidney Foundation, and/or is a member of the Renal Practice Group.
Work Locations

At time of hire, the RDN will work as a “mobile” employee. A “mobile” employee works remotely for the majority of the work week and can accomplish the job fully remotely, with occasional work onsite as needed or requested.
Background Screening

Michigan Medicine conducts background screening and pre-employment drug testing on job candidates upon acceptance of a contingent job offer and may use a third party administrator to conduct background screenings. Background screenings are performed in compliance with the Fair Credit Report Act.
