An established Home Health/Hospice company is seeking caring RN's in the Houston Texas area and surrounding counties. We are an Agency that care about our staff’s health and their families we want to make sure they stay as healthy as possible by using increased safety protocols and increased patient screenings for infectious diseases. We partner with some of the worlds leading safety professionals in establishing safety protocols for our staff, their families, and our patients.
- Currently licensed with the state of Texas
- Excellent verbal and written communication skills.
- Experience with electronic medical records is strongly desired.
- Recent infusion therapy patient care services, delivery and teaching experience, preferably in home health care setting a benefit.
- 1 year of Homehealth experience preferred
- Flexible schedule
- Kinnser experience preferred but not required
- Be available by pager or phone during normal working hours, when on call, and when on back up call.
- Performs patient visits according to the Plan of Care or as patient need dictates.
- Delivers care and instruction that is appropriate to the patient’s.
- Performs required documentation including attempted point of care medical record documentation, inventory records and verbal and written communication with referral sources.
- Coordinate referral process for infusion therapy services with pharmacy, referral sources and subcontracted home care staff.
- Perform case management activities for assigned patients. Responsibilities for case management.
- Participate in the development, implementation, and evaluation of home infusion therapy education protocols and materials.
- Respond to verbal and written issues, concerns, or complaints from home care patients, families, physicians, nurse.
- Comprehensive Assessments of your patients utilizing the OASIS data sets
