Hiram W. Davis Medical Center (HDMC) is recruiting for Benefits Social Worker. This is an excellent opportunity to join a progressive team of professionals in an 94-bed Joint Commission accredited skilled, intermediate, and general-medical hospital. The Benefits Social Worker will complete and submit applications for entitlement programs such as SSI, SSDI, Medicare, and Medicaid to ensure residents and patients have the benefits they are entitled to. This position will also complete other social services duties including tracking the need for PASRRs, completing UAIs for discharge planning, and assisting with re-commitment hearings.

ob Duties:
  • Conducts inquiries for benefits/entitlements for Social Security Administration (SSA), Department of Social Services (DSS), Veteran’s Administration or other agencies as needed;
  • Completes applications for Social Security Administration (SSI, SSDI, Medicare, and Medicare D, Social Services, and Department of Medical Assistance Services (Medicaid, Auxiliary Grants, SNAP, etc) along with any required documentation;
  • Submits applications within timeframes indicated in Social Work Department Policy;
  • Communicates with families as needed to complete benefits;
  • Maintains working knowledge of eligibility criteria, application process and significant time frames for Social Security Administration, Department of Social Services, and Department of Medical Assistance Services;
  • Completes Uniform Assessment Instruments (UAIs) for residents/patients who require them and coordinates with the Health Information Management Department to submit them with the needed Medical Record documentation;
  • Assists with obtaining resident/patient documentation such as state IDs and birth certificates;
  • Assist with re-commitment hearings;
  • Other duties as assigned


KSA’s Required to Successfully Perform the Work:
  • Knowledge of human behavior and development, social systems and social work principles and practice; social work methodology, and case management.
  • Ability to provide social work services to patients; coordinate agency and community resources; communicate orally and in writing all assessments and interventions with patients, families and external agencies; and to work effectively to solve problems.
  • Knowledge of mental health diagnoses, behaviors and intellectual disability.
  • Knowledge and ability to provide social work case management and coordinate resources.
  • Ability to individually develop goals, plans and interventions according to patient needs. Knowledgeable of NASW Code of Ethics.
  • Ability to multi-task with flexibility.
  • Ability to participate on an Interdisciplinary Team and be a contributing member.


Minimum Qualifications
Minimum Qualifications:
  • BSW from an accredited School of Social Work
  • Valid Virginia Driver’s License in good standing with DMV
  • Experience in case management and social work
  • Experience with completing applications for entitlement programs such as Medicaid, Medicare, and/or Social Security.

Preferred Qualifications
Preferred Qualifications:
  • Working knowledge of principles and practices of social casework, working knowledge of the functions of public and private social services agencies as related to health institutions and human affairs services.

Special Requirements
An extensive criminal history background investigation, not limited to employment references, drug testing and E-Verify will be conducted.
