Position Type: Part-Time

Pay Range: Hourly – Min: $22.15 – Max: $36.49 (based on qualifications and experience )

Job Summary:
This position works in collaboration and continuous partnership with chronically ill or “high-risk” patients and their family/caregiver(s), clinic/hospital/specialty providers and staff, and community resources in a team approach. Responsibilities include but are not limited to; promoting timely access to appropriate care, increasing the utilization of preventative care, reduce emergency room utilizations and hospital readmissions, and facilitating patient access to appropriate medical and specialty providers.

Must foster a culture of patient and family centered care, service, quality and safety excellence, while improving patient experience, employee experience and community relations

Required Experience & Skills:
Qualified candidate will have 2 + years’ experience working with patients with chronic conditions, prenatal/perinatal patients, children & families, prior experience with care coordination/care management in a medical setting and prior experience with managing competing priorities.
Must be familiar with community agencies, ability to work independently with internal and external customers
Strong written and verbal communication skills
Able to take and follow through with delegated tasks and accountability
Ability to demonstrate excellent customer service and strong cultural competency
Ability to adhere to strict confidentiality guidelines
Medical terminology

Required Education: High School Diploma or Equivalent,
Degree in Nursing is preferred, LPN or RN preferred.
Bilingual skills preferred
