Summary Job Description:

Reporting to the Nurse Supervisor, the Registered Nurse/Licensed Practical Nurse (RN/LPN) Nurse Case Manager will be accountable for the full patient care cycle while the child is in foster care. The successful candidate will be responsible for developing, implement, and evaluate individualized patient care plans for children on their caseload in foster care. The nurse serves as an advocate and liaison for the children (age range from birth to 21 years old).


This position is part time and requires 3 days onsite.


Principle Responsibilities:

  • Assesses, plans, implements and monitors the overall healthcare of the foster child and coordinates and provides care promptly by child developmental milestones and as needed. NCM encourages good health practices and preventative healthcare with foster parents and child. Provides the child/children with resources that should ensure continued good lifetime practices needed for independent living. NCM also ensures the wellbeing of the individual child and foster parent
  • Conducts assessments, including HIV risk assessments, intake assessments, general first aid and triage activities
  • Conducts routine screening for substance abuse, developmental health, mental health and physical body checks for abuse
  • Provides routine management and training regarding chronic conditions, such as diabetes and asthma
  • Provides training and health education including reproductive health education
  • Provides support and management of the emotional/psychiatric needs of children
  • Establishes treatment goals for children, utilizing historical and currently available information in collaboration with the multi-disciplinary team
  • Maintains medical supplies and equipment
  • Practices proper infection control
  • Follows up after medical appointments, urgent/emergency care or hospitalization
  • As needed, assists with home visits, educating caregivers, and monitoring healthcare needs
  • As needed assists with community provider visits
  • Assists with well-child clinic on scheduled
  • Maintains proper documentation and open communication with medical, case planners, mental health and other specialties involved.
  • Coordinates specialty appointments with community-based resources as needed
  • Other relevant duties as assigned

Special Qualifications:

  • Experience in child welfare system
  • Knowledge and expertise of the foster care healthcare requirements
  • Ability to establish effective working relationships with parents, foster parents, casework staff, pediatrician, schools and community providers and all members of the child's care team to ensure the safety and overall healthcare of children in foster care
  • Computer savvy with MS Outlook software experience at a demonstrated competent level as well as ability to learn/use internal EHR and Connections program
  • Problem-solving skills and ability to multi-task
  • Compassionate, proactive, and motivated personality

Minimum Requirements:

  • Bachelor of Science in nursing (BSN) who is also a registered nurse, registered nurse (RN), RN or licensed practical nurse (LPN)
  • Certification by the American Red Cross (or an equivalent) in BLS with First Aid or the ability to obtain before hire
  • Two years of work experience
  • Bilingual capability - English/Spanish, a plus
  • MCO experience a plus
  • Familiarity with Netsmart a plus
