Position Title: Community Health Worker
Site: 93706 Neighborhood Resource Center, Fresno CA
Department: Policy & Leadership
Rate: $20.00-$21.23
FTE: 37.5 Hours/Week, Full-Time Non-Exempt, Plus Benefits
Reports to: Community Health Worker Supervisor
Location: Fresno County

THE PROGRAM
Centro La Familia Advocacy Services (CLFA) with the City of Fresno and partner community-based organizations is implementing the Advancing Health Literacy program to enhance equitable community responses to the COVID-19 pandemic. The program, focusing on health literacy, will reach community members and increase their understanding of health-related concepts, decrease poor communication between adults and health care providers, and empower adults to be more involved in decisions about their health. The services will operate from a grassroots approach and staff will serve as community outreach workers and educators.

BASIC FUNCTIONS
Community Health Worker (CHW) will provide culturally and linguistically responsive health literacy curriculum to target populations to increase their health literacy related to, but not limited to, COVID-19, health care system navigation and underlying, pre-existing conditions. CHW will serve as advocates targeting high-risk and underserved minority communities. They will work with providers to ensure patients are included in decision-making about their health. Populations will be reached in person, social media, media and other venues. The CHW will track deliverables, meet reporting requirements, attend meetings and trainings, and work closely with CHW Supervisor and partners.

DUTIES AND RESPONSIBILITIES

  • Provide culturally and linguistically responsive health literacy education
  • Implement health education activities in Fresno targeting high-risk and underserved racial and ethnic minority populations
  • Provide education on topics that include COVID-19, chronic diseases, healthy behavior promotion, and disease prevention
  • Reach populations through outreach events, social media, media, presentations and other venues
  • Identify and connect community members with resources
  • Develop and follow the outreach plan ensuring deliverables are met
  • Conduct case management and linkages
  • Track all activities in the program using established tracking tools and case management activities
  • Meet reporting requirements as outlined including evaluation activities
  • Work closely with partners in the program including the City of Fresno and community based organizations
  • Responsible for compliance of mandatory intake forms daily and weekly reports
  • Ensure all requirements and deliverables are met
  • Participate in weekly team meetings and trainings as required
  • Wear personal protective equipment such as face mask, gloves, and follow safety guidelines
  • Responsible for ensuring privacy
  • Performs other related duties as assigned

MINIMUM QUALIFICATIONS

  • Bachelor’s degree in public health, health science, or related field
  • Experience working full time as a Peer Support Specialist, Health Educator, Navigation Specialist or similar field
  • Understanding of the Promotora model
  • Excellent interpersonal skills to assist distressed individuals from different cultural backgrounds
  • Experience working in community outreach, and/or education programs preferred
  • Bilingual a plus, preferably in English/Spanish
  • Access to a telephone, computer, and reliable internet connection
  • Proficiency in MS Word, Excel, and Outlook Express, as well as data entry
  • Ability to comply with regulations and communicate official information in an appropriate manner
  • Advanced ability to coordinate efforts, facilitate intervention, and maintain records.
  • Exceptional organizational skills and sound judgment
  • Valid CA driver’s license and auto insurance

TRAINING AND PROFESSIONAL DEVELOPMENT

  • Satisfactorily complete comprehensive training delivered by COVID-19 Equity Project partners during onboarding and will be responsible for assisting in the training of staff and volunteers as needed
  • Attend mandatory training as part of their orientation and onboarding and will comply with state and federal regulatory requirements related to protected health information and privacy

TYPICAL WORKING CONDITIONS: The office setting is a normal environment. Occasionally work during early morning, evening, or weekend. May me subject to temperature variances outdoor during events.

TYPICAL PHYSICAL DEMANDS: Requires sitting, standing, or walking for up to eight hours a day. Some bending, stretching, or reaching may be necessary. Lifting to 40 pounds may be required on occasion.
Vision must be correctable to 20/20 and hearing must be in the normal range for telephone contact.
