HomeCentris Personal Care Division is seeing a Community Outreach Coordinator to help us grow in the Lanham, MD and Baltimore County surrounding areas.

The Community Outreach Coordinator is primarily responsible for sourcing, establishing, and maintaining relationships with client referral sources and prospective clients. The Community Outreach Coordinator is the face of HomeCentris in the local market. The position is responsible for providing outreach and education regarding the Agency’s services to potential clients and referral sources and bringing new clients into the Agency.

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES

• Develop new referral sources by planning and organizing schedule to call on potential referral outlets and other sources of prospective clients.
• Meets with prospective clients in their homes to discuss agency’s services, payment options, onboarding process.
• Assists the agency’s Client Relationship Managers in processing and on-boarding new clients.

• Communicates with and supports existing clients as needed in conjunction with PHC’s Client Relationship Managers.

• Keeps management informed by updating CRM systems, submitting sales activity and results reports, such as daily call reports, weekly work plans, and monthly and annual territory analyses.

• Develops “out of the box” sales strategies/models and evaluates their effectiveness.

• Assist with developing recruitment strategies to attract candidates.
• Collaborates closely with Recruiting and Human Resources to ensure candidates complete onboarding process.
• Other duties as assigned.
