Midway Nursing Home is seeking a strong Director of Social Work. The ideal candidate will have exceptional communication and organizational skills.

Duties Include:
Direct, establish & plan the overall policies/goals for the Social Services Dept

Responsible for completion of assessments, MDS and care planning

Responsible for scheduling of and leading CCP meetings

Coordinating discharges for the residents to the community. Assists residents with finding appropriate housing, arranging for appropriate equipment for discharge back into the community.

Hospital / Community Outreach

Perform audits in response to reportable events and performance improvement plans.

Rely on extensive experience & judgment to plan and accomplish goals

Must Adhere to all HIPA REQUIREMENTS.

Educate the residents and responsible party regarding rights related to advanced directives.

Assist the residents/responsible party and coordinate with nursing and the attending Physician the resident/responsible party regarding the resident's wishes and expectations for advanced directives/code status.

Assess and provide support for adjustment to a new room and/or new roommate.

Review complaints and grievances made by the resident and make a written or oral report to the Administrator indicating what actions were taken to resolve the complaint or grievance.

Ensure that all new residents/family /responsible party on admission have been given residents rights.

Review the clinical Dashboard daily for due and outstanding, Care Plan reviews, draft notes and high risk progress notes.

Maintain written and verbal communication on resident discharges to the community with the Interdisciplinary Team.

Qualifications/Requirements:
Prior experience as a Director of Social Work in a nursing home setting a plus.

Must have BSW or MSW

Knowledge of state and federal regulations

Must be a self-starter and energetic

Proven Leadership & Management Skills

Strong verbal & written communication skills

Knowledge of Visual and Sigma EMR a plus

Salary commensurate with experience
