For individuals with developmental disabilities, housing can sometimes be an issue for various reasons and our Enhanced Family Care team helps address the issue. They find and train community members who are willing to open their homes to one or more individuals with disabilities. As an Enhanced Family Care (EFC) Coordinator, you are responsible for the development, maintenance, and support of quality Adult Foster Care (Home Providers) services which include finding appropriate homes/matches, monitoring and managing contracts, reviewing provider certification compliance as well as providing training and general oversight. You model and ensure advocacy and support for individuals while working to expand community opportunities.

JOB FUNCTIONS AND RESPONSIBILITIES:

  • Develops, maintains, and supports residential living for both individuals and Home Providers
    • Services as primary contact for Home Providers
    • Coordinates with various Program Managers and Case Managers as needed to ensure quality of care
    • Oversees EFC Contracts and monitors contractual obligations and compliance
    • Ensures the rights of individuals are maintained and is responsible for reporting any suspected violation of rights, as outlined in the agency policies and procedures.
  • Develops both current and new Home Providers
    • Recruits new Home Providers through advertising and referrals
    • Provides and oversees training for Home Providers on regulations, policies, and individual-specific requirements
  • Attends and participates in all meetings pertaining to an individual and Home Provider as needed.
  • Oversees monitors quality assurance of residential living:
    • Ensures Home Providers have the required and necessary information and forms for documentation purposes and the necessary training on the completion of documents
    • Oversees and monitors Home Provider certification process
    • Oversees and monitors Home Provider adherence to policies and procedures including documentation
    • Oversees and monitors service delivery from Home Provider meets the requirements of the individual’s Service Agreement.
    • Documents regular quality assurance activities including every other month Home Provider visits. Maintains agency records for each contracted EFC provider.
  • Ensures that the health and safety of the individuals are maintained:
    • Ensures EFC providers maintain a high standard of care.
    • Monitors household composition and physical environment.
    • Reviews life safety systems to ensure they are operational.
    • Remains prepared for emergency response and placement as needed
  • Participates in ON-CALL rotation as scheduled and shares coverage of others as requested during vacations, sick time, emergencies, or other times as assigned.

REQUIREMENTS:

  • Associate’s Degree in human services or related field required.
  • 3 years of human services program experience is a must
  • Proficiency in MSOffice (Word, Excel, Outlook) required
  • Must possess strong verbal, reading, and written communication skills.
  • The ability to work effectively and harmoniously with others is necessary. Must have a willingness/ability to communicate with and relate to individuals, parents, guardians, co-workers, representatives of community agencies, and agency management.
  • Must be able to work a flexible schedule including evenings, weekends, and overtime as required to meet agency needs.
  • Experience and/or willingness to work with forensic and safety issues a must; experience with challenging behavioral issues is highly preferred
  • Must have and maintain a valid driver's license, proof of personal automobile liability insurance coverage, and access to a reliable vehicle.
  • Must pass TB Test
  • Must be fully vaccinated for COVID prior to start date
