Facility Rehab Coordinator (FRC)

We are currently seeking a committed and patient-focused Facility Rehab Coordinator (FRC). The Facility Rehab Coordinator of the rehabilitation department will lead a dynamic multidisciplinary team of physical therapists, occupational therapists, and speech therapists in the skilled nursing facility.

  • Excellent interdisciplinary communication skills

  • Proficiently collaborate with facility administration
  • Should have a fundamental understanding of PDPM reimbursement.

Coordinates, leads, and directs facility rehab department. Provides clinical expertise in treatment of patients. Serves as a role model to rehabilitation employees by providing effective and efficient direct patient care. Expands and updates clinical skillset. Provides direct patient care in specialty area when needed.

Responsibilities:

  • Assumes responsibility for the efficient and profitable management of rehabilitation services in assigned account(s).
  • Manages designated locations rehabilitation staff.
  • Maintains a work environment that fosters quality care delivery, accountability, and employee satisfaction.
  • Manages development and implementation of appropriate standards of care across assigned therapy locations.
  • Ensures delivery of rehabilitation services emulates "best practices" and is in compliance with regulatory and accreditation standards.
  • Assumes responsibility for daily staffing and utilization.
  • Administers financial controls of revenue and expenses.
  • Assume responsibility for facility reports on a weekly and as needed basis.
  • Assists in the recruitment and retention of Arbor Rehabilitation employees in assigned accounts, along with Area/Region Manager and Recruiter.
  • Supports the delivery of the highest level of clinical care within assigned account(s) through appropriate utilization of resources, the promotion of clinical programs and staff development.
  • Ensures that the clinical management of the rehabilitation department, including oversight related to case management, care planning, appropriate delivery of therapy utilization and patient identification.
  • Ensures compliance with company policies, procedures and practices within assigned account areas and reports variances.
  • Collaborates to deliver clinical services that result in positive clinical and financial outcomes with high levels of customer service.
  • Implements and manages performance and process improvement initiatives, productivity benchmarks and standards, approved budget, and financial management activities.
  • Provides direct patient care in specialty area.
  • Other duties as assigned.

Requirements:

  • Current state licensure as a Physical Therapist (PT), Physical Therapy Assistant (PTA) Occupational Therapist (OT), Occupational Therapy Assistant (OTA) or Speech-Language Pathologist.

Complexity of Work:

  • Requires critical thinking skills, decisive judgment, and the ability to work with minimal supervision.
  • Must be able to work in a stressful environment.
  • Excellent observation, verbal and written communication skills, critical thinking skills, basic math skills.
  • Compassion, patience, and a positive attitude

Required Experience:

  • Skilled Nursing Facility experience preferred.
  • Previous therapy management experience preferred.
  • PDPM experience preferred.
