Great opportunity for a motivated FNP looking to make a difference at the community level, and work with a team in an outpatient setting to a multicultural patient population and help patients to achieve the best level of wellness. Candidates must have a current California FNP license. New graduates are welcome to apply. Individuals will work in a Patient Centered Medical Home Model, and work closely with a team of professionals to provide culturally sensitive, high quality, comprehensive health services in our Federally Funded Community/Migrant Health Center.

FNP provides quality care to all of the patients at SBHF, participates in quality improvement, prevention activities in the community and health disparity reduction projects, and Collaborates with those responsible for case management, social services, referrals, outreach, community prevention and other SBHF managers.

Applicants must meet the following criteria:

• NP Degree from a recognized University
• 1-2 years’ experience working in a primary care clinic setting preferred
• Current and Valid NP License
• Current and Valid DEA License
• Bilingual/Spanish is a plus
