NOTE: The California Department of Public Health (CDPH) has issued new requirements for health care and congregant care facilities to decrease the risk of COVID-19 outbreaks. Based on the CDPH public health mandate, all candidates for positions at Edgewood Center must provide proof of vaccination. Family Partner II- This is a full-time position, non-exempt, (40 Hours), Mon.-Fri., 8:30am-5pm with some evenings. It is a hybrid position/office/telehealth with community based work.

Program Description
The Turning Point C/Y Program is a comprehensive, full service partnership (FSP) for 40 of the highest risk children/youth living in San Mateo County. Turning Point C/Y is designed to help children and youth achieve independence, stability and wellness within the context of their culture, community and family/caregiver. Turning Point C/Y seeks to stabilize youth in the community using natural support structures.

Position Description
The Family Partner II is a formal member of our multidisciplinary team whose role is to serve parents, caregivers, and non-professional supportive adults of our youth (6-21 year old's). Family Partners use their own personal experience as a family member of a child or young adult who has special needs who has an understanding of the multiple child and family serving systems, these may include: schools, mental health, physical health/medical, juvenile justice and/or legal, child welfare systems. Family Partners build relationships with parents and caregivers in order to offer support, coaching, advocacy and linkage to community and family supports and services. In addition Family Partners coordinate and facilitate support groups and skills groups for caregivers, family members and any other individuals our transition age youth consider "family."

Essential Functions
  • Provide counseling and behavioral interventions, guidance, skills-building and support in accordance with each client's individualized treatment plan and the program's treatment philosophy and model.
  • Provide services to 6-21 year old's and their families at Edgewood locations, in the home, at school and/or in community settings, as required by the treatment plan.
  • Engage family members and caregivers as full partners in service planning, delivery, and evaluation.
  • While under the Family Partner's care, maintain supervision of clients at all times to ensure their safety and well-being.
  • Ensure facilities, living quarters, office spaces, equipment and furniture to be used in providing services are in good working order and professionally maintained at all times.
  • Engage in crisis interventions and de-escalation, including the application of conflict resolution and collaborative problem solving technique.
  • Implement clinical policies and procedures and actively enhance communication, coordination and integration of care with other Edgewood service areas and with external providers.
  • Contribute to organization-wide quality management and improvement efforts by consistently providing the highest quality professional interventions, fulfilling all documentation requirements, and proactively seeking ways to enhance the quality of services and surpass minimum required standards.
Applied Knowledge, Skills and Abilities
  • High School diploma or equivalent required; Bachelor's degree preferred. Equivalent education and relevant experience may be considered in lieu of a Bachelor's degree.
  • Demonstrated ability to work with families in a wide variety of cultural contexts and community settings.
  • Must have a valid California driver's license and proof of insurance.
This position offers an hourly rate of pay of between $24.75. to $27.23 per hour depending on experience. The hourly range for bilingual ( Spanish/English) candidates is $25.00 to $26.00 per hour.
