JOB DESCRIPTION:
Responsible for all direct care staff on the assigned shift. He/She is to provide direction, supervision, and evaluation of resident care.
Implements facility policies, procedures and coordinates nursing activities with other services provided by members of the health care
team. Under the supervision of a Director of Nursing, Assistant Director of Nursing or Registered Nurse performs hands-on nursing
care for the residents with emphasis on daily care needs, personal hygiene and cleanliness, grooming and skin care. The staff member
must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or
her assigned unit. The individual must demonstrate knowledge of the aging process, possess the ability to assess the patient's age specific
needs, and provide the care needed.

QUALIFICATIONS: Current State License registration
CERTIFICATION: Issued by the State
CONTINUING EDUCATION: As required by the state and facility, including seminars.
EDUCATION: Graduate of an accredited school of nursing.

DUTIES AND RESPONSBILITIES:
1. Follows established standards of nursing practice and implements facility policies and procedures on the assigned unit.
2. Interprets existing policies and procedures to nursing assistants, restorative nursing assistants, residents, families and physicians.
3. Supervises and evaluates all direct resident care provided and initiates corrective action as necessary.
4. Obtains report from nurse he/she is relieving and records sufficient information for follow up action as necessary.
5. Provides report to nurse coming on duty, including sufficient information for follow up action as necessary.
6. Conducts resident rounds daily, reports problems to nursing supervisor and initiates corrective action promptly.
7. Identifies safety hazards and initiates corrective action immediately.
8. Identifies resident problems and emergency situations and initiates immediate lifesaving measures in the absence of a physician.
9. Supervises and evaluates implementation of the “Resident Rights.”
10. Participates in facility education programs and coordinates student activities with the director of nursing education.
11. Provides clinical supervision of nursing assistants providing direct resident care.
12. Attends staff meetings, in-service classes and committee meetings as assigned.
13. Follows staffing guidelines and makes nursing assistant’s assignments according to the qualifications of staff and characteristics of the resident population.14. Uses supplies and equipment efficiently and effectively.
15. Assumes responsibility for compliance with federal, state, and local regulations within the assigned unit.
16. Counsels nursing assistants and recommends disciplinary action, when necessary, to the Director of Nursing.
17. Participates in the employee evaluation process by providing information on the clinical performance of nursing assistants to the Director of Nursing.
18. Administers medications and treatments per the physician’s order and accurately records all care provided.
19. Accurately, completely, and timely documents resident care provided and residents’ response or lack of response to care provided.
20. Reports changes in residents’ conditions to a physician, director of nursing, and responsible party promptly and takes follow-up action as necessary.
21. Follows facility admission procedures and communicates resident admission to other departments, physicians, and ancillary service providers.
22. Follows facility discharge/transfer procedures and coordinates discharge/transfer with other departments, physicians and ancillary service providers. Completes accurate, timely documentation related to the resident’s discharge/transfer.
23. Demonstrates concern for all residents’ call for assistance.
24. Controls angry feelings appropriately.
25. Demonstrates sensitivity to feelings of others by listening and taking follow up action as necessary.
26. Supervises prompt response to residents’ call for assistance.
27. Greets residents by name and speaks to residents and family members, demonstrating a kind, thoughtful manner.
28. Listens to resident, family, or physician concerns attentively and initiates corrective action and/or
reports concerns to nursing supervisor promptly.
29. Works where assigned with a cooperative spirit.
30. Comes to work as scheduled, on time, and consistently demonstrates dependability and punctuality.
31. Comes to work in a neat, clean uniform and consistently present a professional appearance.
32. Assumes personal responsibility for obtaining continuing education related to long-term care nursing.
