POSITION PURPOSE:

The Outreach Worker will conduct CHI outreach enrollment of eligible residents for health care insurance in Medicaid/KidCare or The Exchanges.

POSITION REQUIREMENTS / QUALIFICATIONS:

Education/Experience:
High School Graduate or GED Equivalent.
Previous experience with outreach desired.

Licensure / Certification:
Maintain current CPR certification from the American Heart Association. Valid Florida Drivers License.

Skills / Ability:
Good communication and interpersonal skills. Sensitivity in working with diverse cultural groups bilingual preferred.

POSITION RESPONSIBILITIES (THIS IS A NON-EXEMPT POSITION)

Attends CHI orientation and all scheduled trainings by State/Local organizations to learn all aspects of the Affordable Care Act.

Attend training at DCF for Medicaid/KidCare and PEPW.

Attend trainings on in-reach and out-reach strategies.

Assist patients with information on how to access a medical home.

Attend meetings as scheduled with collaborators.

Help in implementing a campaign for the program.

Identify uninsured CHI patients and also non-CHI residents.

Assist with identifying and removing barriers to making and keeping appointments, e.g., transportation needs, insurance, language, enrollment, education.

Conduct screening, education and enrollment in the community with the tools provided as needed.

Help uninsured and underinsured patients apply for benefits such as Medicaid, KidCare, Medicare or health Exchange.

Ensure enrollment in health insurance plans as applicable.

Follow up within 24 hours of referrals from the Navigator, partners or collaborators.

Conduct outreach during non-traditional hours.

Contact patients through telephone calls, text message, home visits or letters.

Collaborate with existing programs and departments to ensure program compliance.

Act as a liaison between CHI and the community.

Assist people with information on how to access a medical home cross sell within all CHI departments, use sales skills.

Maintains accurate daily logs with all contact activities and contacts.

Maintains accurate records of the activities performed and posts all activity daily.

Complete event logs of all community activities and disseminates the program literature.

Assist with follow-up of patients who need to provide documentation.

Attends scheduled CHI in-services and annual updates.

Maintains regular contact with patients and non CHI patients to ensure their needs are met.

Participates in Performance Improvement Programs as required.

Promptly informs the supervisor of problems encountered in the line of duty.

Follows CHI Policies and Procedures.

Meets the monthly program goal and submits monthly reports.

Maintains 80% productivity monthly.

Demonstrates respect and regard for the dignity of all patients, families, visitors and fellow employees to ensure a professional, responsible and courteous environment.

Continuously displays a positive attitude within the department and cross departmental lines to contribute to the overall customer service program in place in CHI.

Answers the telephone following CHI’s standards for telephone etiquette.

Completes B&E (billing and encounter) forms within the same day of intervention.

Reports to work on time and ready to work with minimal absenteeism.

Submit travel request before the 10th of the following month.

Comply with CHI dress code as required.

Maintain high standard of customer satisfaction to include the use of AIDET at every encounter.

Adheres to Confidentiality Policies and Procedures/HIPAA Regulations.

Performs other duties as assigned.
