Job Summary: This is a professional level position involving the supervision of the direct care staff.

Supervisor: Lead Qualified Professional

Duties and Responsibilities: The work duties and responsibilities of the position of Qualified Professional (QP) include, but are not limited to, the following:

  • Supervise, train and evaluate assigned direct care staff working with individuals served by Reach who are enrolled with the Innovations Waiver program.
  • Evaluate the consumer’s needs.
  • Participate in the development of the Individualized Support Plan for all individuals served assigned.
  • Develop habilitative goals with consumer and/or consumer’s family.
  • Monitor progress and revise goals with consumer and/or consumer’s family.
  • Provide family home visits to ensure program compliance and family/consumer satisfaction and/or schedule face-to-face visits with staff in all service locations.
  • Assure consumer/family confidentiality.
  • Monitor and submit all required documentation in a timely manner.
  • Report the need for protective services of a child/adult with disabilities due to suspected abuse, neglect, or exploitation.
  • Receive continuing education on issues pertaining to persons with disabilities.

  • Attend Innovations waiver training classes and/or provider meetings as directed by the Reach President.
  • Monitoring and updating consumer files
  • Monitoring and preparing AFL privileging files for audits and compliance
  • Printing and calculating client authorizations
  • Other projects and responsibilities may be added at the discretion of the agency

All job requirements will be the sole responsibility of the employee to obtain and, if applicable, keep current. Furthermore, any training and qualifications below, whether mandatory or preferred, will be at the employee’s own personal expense.

Training and Qualifications:

  • Graduation from a four year college or university, in a discipline related to developmental disabilities, with at least one year of supervised habilitative experience; or, graduation from a four year college or university in a human service field and two years of supervised habilitative experience working with individuals with developmental disabilities; or,graduation from a four year college or university in a field other than one related to developmental disabilities and at least four years of full-time, post-bachelor's degree accumulated MH/DD/SAS experience with the population served; or, a registered nurse who is licensed to practice in the State of North Carolina by the North Carolina Board of Nursing with four years of full-time accumulated experience in MH/DD/SA with the population served.
  • Knowledge of fundamental principles, techniques, and methods in developmental disabilities which include experience in the assessment and development of outcomes and activities.
  • Current CPR/First Aid Certification.
  • Current Blood Borne Pathogens/Universal precautions training.
  • Must successfully complete Core Competencies and required refresher training.
  • Satisfactory criminal record check.
  • Satisfactory Healthcare Registry check.
  • Valid North Carolina driver’s license, satisfactory driving record check and acceptable level of automobile liability insurance.
  • Provide own transportation to perform any duties above.
  • Ability to be flexible.
  • The supervisor and the employee shall develop and individualized supervision plan upon hiring. The parties shall review the plan annually.

Job Type: Full-time

Pay: $35,000.00 - $37,000.00 per year
