Position Overview:

RNs are responsible for traveling to patients’ homes to assess, plan, coordinate, provide and document care of the patient from day of admission through day of discharge. They utilize physicians’ orders and professional skills to implement a plan of care that meets each patient’s specific needs. They work 1 on 1 with their patients; collaborating and coordinating with our entire care team. They also participate in case conferences, as well as, continuing education/skills courses . All while maintaining compliance with current standards and regulations.

Licenses & Certifications

• Current RN License (Utah)
• CPR Certification
• Driver’s License
