Job description

We need an RN OR LPN that has at least one experience in a community setting. We are a small Day Habilitation Program located in Wellesley. It is a day shift only Monday-Friday for 6 hours per day (30 hours a week).

• The nurse will work with members daily,
• monitor health, administer medications, write monthly notes, semi annual assessments, and develop Care Plan and etc.
• Directly oversee health and safety practices for program participants
• Ensure compliance with Dept. of Medical Assistance and CARF national accreditation requirements.
• Provide routine medical treatment and emergency treatment, such as first aid, as necessary.
• Provide education and training in health and safety matters.
• Provide direct care and public health education and services in all areas.
• Maintain all nursing, health, accident records/reports for each individual.
• Obtain reports and approval of treatment plans from primary care physicians.
• Coordinate day habilitation plans with other health care professionals.
• Monitor each day habilitation participant’s health status.
• Acts as a liaison to participant physician and/or medical clinic.
• Implementation of physician’s orders.
• Participate in case conferences/ISP’s to review participant status and needs.
• Advise Program Director, Project Director and staff of medical problems that may hinder a participant’s progress.
• Participate in Semiannual reviews for each participant in day habilitation as well as complete monthly nursing notes.
• RN Supervises and evaluates LPN
• Additional duties as assigned (e.g., Zoom calls)

Qualifications: Current Massachusetts license to practice as a Registered nurse. First Aid and CPR Certifications. Experience with Dept. of developmental Services regulations and service delivery systems preferred. One-year experience in community health setting. Patience, dedication and love.
