Federations of Organizations is seeking Case Managers for the Residential Department. There are several positions available within different boroughs of NYC (Queens, Brooklyn, Bronx)

Job Function:

Provide case management services, housing supports and coordination of services to people with psychiatric disabilities living in scattered apartment settings.

Qualifications:

High School Diploma and at least 3 years experience in mental health. BA in Human Services and 1 year experience preferred. Clean valid NY State Driver’s License. Fingerprinting, criminal record check, and approval from NYS Office of Mental Health.

ESSENTIAL FUNCTIONS:

A. Prepare support plans as scheduled with residents quarterly.

B. Maintain all aspects of resident’s records including admissions, progress notes, discharges, follow-up contacts, etc., on a daily basis.

C. Maintain rapport and communication with residents’ treatment teams and support network quarterly or as often as required.

D. Provide linkage with out-patient services as needed each month.

E. Advocate with social service agencies to ensure entitlements are secured and maintained as often as necessary each month.

F. Provide transportation to residents as needed each month to community resources (such as banks, social service agencies, psych and/or medical appointments, etc).

G. Work collaboratively with the SSS team including the intervention specialist for residents with high risk issues on a daily basis.

H. Attend weekly staff meetings, monthly high risk meeting and monthly trainings as scheduled and actively participate in each.

I. Report all incidents or emergencies to program supervisor on a daily basis.

J. Locate and set up new apartments, i.e., furniture, household goods, contact utility companies, etc., in collaboration with the facilities management specialist as often as necessary each month.

K. Prepare rental worksheets/income verifications as required annually or as often as needed as benefits or income changes.

L. Negotiate landlord/tenants relations and any community problems as often as necessary each month.

M. Provide on-call crisis intervention during after hours, weekends and holidays on a rotating basis.

N. Utilize collaborative documentation in your daily practice.

O. Complete reports as often as necessary each week.

P. Comply with agency Compliance Plan and False Claims Policy by preventing, detecting and reporting any potential healthcare abuse and fraud. Report any potential non-compliance and comply with all other federal, state and local regulations applicable to this job description.

Q. Utilize all agency-based IT applications in a proficient manner including collaborative documentation, review of emails at least twice per shift (at start and prior to close of the shift), as well as utilizing Outlook Calendar on a daily basis.

R. Remain current on changing developments in individual field of expertise and licensing credentials.

S. Perform all other job duties as assigned
