The Thresholds Homeless Outreach Program (HOP) was established in 1989 to provide assertive outreach, linkage to primary and psychiatric care, and resource referral to persons experiencing homelessness with serious mental illnesses. The program employs the skills of licensed clinical social workers on the streets, in shelters, and any other location where people who are homeless may be found. We are Chicago’s only citywide mental health homeless outreach program, and our primary goal is simple: to get people living with mental illnesses off the street and help them recover and regain their lives. We believe it is not enough to simply be available to the people who need our services – we need to bring our services to them.

Team Leader

The Team Leader is a part of the management structure of the program. The Team Leader is responsible for working with homeless individuals who live outside.

Essential Duties & Responsibilities

  • Provides street outreach, case management, job support, crisis intervention, and psycho-social rehabilitation services either in the community or at Thresholds locations.
  • Assesses the need for service, investigates current and available resources, and makes recommendations for treatment.
  • Supports members in efforts to connect with other providers and services, such as Social Security, medical providers, the Chicago Housing Authority, etc.
  • Canvasing areas near the CTA where homeless individuals may reside.
  • Approach homeless individuals and providing them with resources.
  • Helping people make meaningful change.
  • Helping homeless individuals pursue housing, medical, and psychiatric help.
  • Completes all documentation within assigned time frames.
  • Assists with special tasks as assigned, including but not limited to representing the agency at meetings, conferences, supervising students/interns, orienting and training new staff.
  • Conducts annual evaluations in a timely manner for all team staff. Develops specific goals and objectives and provide the method and opportunity for each team staff to succeed in their professional development. May supervise subordinate clinical/administrative levels 6-10 of the assigned team, program, and/or department.
  • Participates in the hiring sequence of the agency and make recommendations on the hiring or termination of team staff.
  • Follows agency standards regarding clinical supervision.
  • Assists Program Director with the development and monitoring of Performance Improvement Plans for staff not consistently meeting agency standards.
  • Participates in the implementation of Thresholds emergency plans related to extreme weather or other emergency condition.
  • Works flexible schedule including work on weekends or evenings as required. May be required to be on-call.
  • Other duties as assigned.

Requirements

  • Willing to work Monday thru Friday, 8:30AM-5PM
  • Must be at least 21 years of age
  • Bachelor's degree preferred
  • Previous outreach experience (preferred)
  • Lived experience (preferred)
  • Valid driver’s license required

Education

  • Must, at a minimum, meet Office of Mental Health rule 132 standards as a Mental Health Professional (MHP)
  • Must have a Bachelor’s degree, or have a Practical Nurses License, and/or experience* working with person diagnosed with mental illness.
    • *Five years of supervised experience in mental health of human services may be substituted for formal education with the approval of the Program Director.

Experience

  • One (1) – two (2) years clinical experience
  • Leadership experience preferred
  • Computer proficiency, including the ability to use email, navigate and enter notes into electronic medical records and working knowledge of pertinent software.
  • Experience at a homeless shelter or drop-in centers
  • Comfortable with working outside
  • Strong interpersonal skills and knowledge of group dynamics required
