TITLE: Aftercare Worker

SUMMARY:
The Aftercare Worker works to support families who have been successfully discharged from DHS/CUA
formal case-management services by providing reintegration supports, consultations and community resources
that will decrease the probability that the family will re-enter the child welfare system. This position involves
working closely with all members of the assigned case management team prior to successful discharge; with
children, biological families, and with external agencies and partners. The Aftercare Worker will focus on
planning intended to prevent re-entry. Tasks will include direct work with families to address needed ongoing
community supports after formal DHS/CUA case-management services are discharged, as well as work with
external partners and systems to ensure that children and families are successfully connected to needed
reintegration and support services.

MINIMUM QUALIFICATIONS:
Bachelors Degree in Social Work or related field required.
Candidates with a degree in an unrelated field require a minimum of three years experience working in
child, youth, and family serving systems.
Must have strong interpersonal skills and the ability to communicate well orally and in writing.
Must have a valid drivers license.
