Job Description/Summary

The case manager is the primary contact for each program participant and has tremendous influence in shaping how each participant views the Connect to Home intervention’s services and benefits. Candidate qualifications must include a strong background in understanding and applying principles governing trauma-informed care, positive youth development, protective and promotive factors, harm reduction, and best practices in supportive housing.

Essential Job Function

  • Maintains individual caseload of 8 clients;
  • Monitors clients and maintains progress reports on attendance, and goal achievement;
  • Brings issues and problems regarding groups to supervisor or staff meetings on a regular basis
  • Makes home visits, assists assigned clients with obtaining housing and securing their housing voucher
  • Contributes to program development and planning;
  • Covers program activities, as needed;
  • Is active in team meetings, asks for clarification, asks questions, shares option, and informs;
  • Shares in paperwork tasks during meeting;
  • Reflects enthusiastic and positive attitude;
  • Takes leadership role;
  • Participates in agency teams and committees;
  • Demonstrates effective listening skills;
  • Is respectful when speaking and interacting with consumers;
  • Effective in setting limits;
  • Able to express personal feelings, issues relating to clients which may affect relationship with consumers, i.e. counter-transference;
  • Takes initiative in reporting to supervisor or team relevant issues regarding consumers;
  • Completes work efficiently;
  • Takes advantage of educational opportunities, i.e. conferences, seminars;
  • Uses resources to increase knowledge, i.e. reading materials, staff expertise, educational videos

Competencies

Attends all team meetings; attends mandatory orientation and trainings; demonstrates ability to perform basic computer functions as required by job function; performs job responsibility with a minimum of supervision from Supervisor; follows policies and procedures outlined in Employee Handbook; attends at least two agency held quarterly meetings per year; demonstrates familiarity with organization and program specific policies; all other duties as assigned.

  • Travel throughout Monmouth and Middlesex Counties - Company Car Provided
  • Must be able to work flexible hours to meet the needs of the consumer
  • Dependability - Instills full confidence;
  • Ability to use time management skills effectively;

Qualifications

  • Working with young adults
  • Knowledge of housing vouchers
  • Driver's license in good standing
  • Experience 3 years working with children, youth or families

Education

  • Bachelors Degree in a social service related field
