DESCRIPTION OF WORK

This is advanced professional social services work for Crawford County Human Services. The primary responsibility is to serve children and adults who have been diagnosed with serious and persistent mental illness and who are in need of multiple services. An important aspect is to provide intensive case management as a primary mode of service which will assist the consumer in gaining access to resources such as medical, housing, social, educational, vocational, and other services. The intensive case management services must be goal directed to include appropriate hospitalizations and enhance quality community tenure and independent social functioning.

Work Schedule and Additional Information:


  • Full-time employment.
  • Work hours are 8:30 am to 4:30 pm, Monday - Friday, with 30-minute lunch.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS
Minimum Experience and Training Requirements:
  • Two years of experience as a County Caseworker 2; or
  • A bachelor's degree in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and three years of professional social casework experience in a public or private social services agency; or
  • Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences.
Other Requirements:
  • PA residency requirement is currently waived for this title.
  • You must be able to perform essential job functions.
