General Summary of Position


This is a new hybrid Collaborative Care Manager position which will cover existing practice at World Bank and new CoCM services at MMG Spring Valley and McLean. The position requires one day in person per week at each World Bank and Spring Valley.

Functions as a core member of a collaborative care team that involves the patient's primary care provider, psychiatric consultant, and other mental health providers. The Care Manager is responsible for coordinating and supporting mental health care within the care setting and for coordinating referrals to clinically indicated services outside the care site.



Primary Duties and Responsibilities

  • Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards, and safety standards. Complies with governmental and accreditation regulations.
  • Completes relapse prevention plan with patients who are in remission.
  • Documents patient progress and treatment recommendations in the registry so they can be easily shared with PCPs, psychiatric consultants and other treating providers.
  • Facilitates patient engagement and follow-up care. Tracks patient follow-up and clinical outcomes. Documents encounter in a registry and use the system to identify and re-engage patients.
  • Facilitates referrals for clinically indicated services outside the primary care clinic such as social services, housing assistance, vocational rehabilitation, mental health specialty care, or substance abuse treatment.
  • Facilitates treatment plan changes for patients who are not improving as expected in consultation with PCP and psychiatric consultant. This may include changes in medications or psychosocial treatments with appropriate referrals for additional services.
  • Monitors patients in person or by phone for changes in clinical symptoms, treatment side effects, or complications.
  • Participates in regularly scheduled caseload consultation with the psychiatric consultant and communicates resulting treatment recommendations to the patient's primary care provider. Consultations will focus on patients new to treatment or those who are not improving as expected.
  • Performs other duties as assigned.
  • Provides brief interventions using evidence-based techniques such as Behavioral Activation, Problem-Solving treatment, Motivational interviewing or other treatments appropriate for primary care settings.
  • Screens and assesses patients for common mental health and substance abuse disorders. Provides basic education and available treatment options. Provides referrals to evidence-based psychosocial treatments as clinically indicated.
  • Supports and closely coordinates mental health care with the patient's primary care provider and, when appropriate, other mental health providers.


Minimum Qualifications
Education

  • Master's degree in Psychology, Social Work or related field required and
  • BSN or LPN with mental health training/certification may be substituted required

Experience

  • 1-2 years Related experience in a similar care model preferred and
  • Experiencing working with patients who have co-occurring mental health, substance abuse and physical health problems. Must have experience with screening for common mental health and/or substance use disorders patient assessments and treatment planning required and

Licenses and Certifications

  • LCSW-C (Licensed Certified Social Worker-Clinical), LMSW (Licensed Masters Social Work) or LCPC (Licensed Clinical Professional Counselor) required and
  • BSN or LPN with mental health training/certification accepted required and
  • PhD in Psychology may be substituted required

Knowledge, Skills, and Abilities

  • Strong communication skill.
  • Demonstrated ability to collaborate effectively in team setting.
  • Ability to effectively engage patients in a therapeutic relationship.
  • Working knowledge of differential diagnosis of common mental health and/or substance use disorders.
  • Knowledge of psychopharmacology for common mental health disorders.
  • Basic computer skills preferred.
