Pay Range: Hourly – Min: $16.50 – Max: $36.49 (based on qualifications and experience)

Job Summary:
Position is responsible for performing nursing functions and support outreach events. This includes coverage for various providers as needed based on the staffing needs of all VW clinics. The primary responsibility of the position is to increase the level of utilization of clinic services and human service resources through outreach to the general community with special consideration for the agricultural worker patient population. The outreach nurse will develop and implement weekly schedule for outreach activities to identify and address community and agricultural worker health care needs, i.e. screenings, home visits, health fairs, and community education events. Connect patients with case management and financial eligibility, and provide referrals to other (community, state, and federal) services as necessary. Assist patients with scheduling health care appointments and provide appropriate follow up to ensure patient compliance.

Candidate must be willing to travel and work a flexible schedule that may include some weekends and evenings.

Must foster a culture of patient and family centered care, service, quality and safety excellence, while improving patient experience, employee experience and community relations.

Required Experience & Skills:
Qualified candidate will be a licensed RN or LPN in Colorado or a Certified Medical Assistant
Bilingual skills are preferred.

Health care experience; lab facilities and service
Medical skills, equipment, and technology used in the nursing area.
Proficiency using the internet and basic knowledge of MS Office, Excel
Good interpersonal skills and ability to work well with the public.

Required Education: Degree in Nursing or Certificate in Medical Assisting

Certifications:
Colorado LPN or RN License or Certified Medical Assistant
BLS certification
