The Enrollment Nurse will work directly with hospital staff to identify
potential patients and screen for their eligibility to participate in the
Biofourmis program. Primary responsibilities include administrative tasks,
such as collecting medical information and ensuring that all eligibility criteria
have been met. Once a patient has been screened and is eligible, the
Enrollment Nurse will work with hospital staff to obtain an order for the
patient to participate in the program, educate the patient about the program
and obtain consent, and complete a bedside enrollment utilizing teach back
methods.
Responsibilities:
- Increase client enrollment to meet contractual obligations
- Maintain onsite presence at hospital to encourage enrollment
- Identify new opportunities to increase enrollment through additional
- Manage patient enrollment from identification through enrollment
- Communicate with Biofourmis care team members to ensure warm
- Work with client to identify needs and translate into solutions that
- Be a strong team member that facilitates collaboration and
- Success working cross-functionally in influencing teams and building
- Deep experience interfacing with interdisciplinary teams
- High degree of user empathy and ability to balance advocate for the
- Passion for making healthcare better for patients and more efficient for
- Patience to solve hard problems and work around operational
Requirements and Qualifications
- Associate degree in nursing; bachelor's preferred
- State appropriate RN license (New Jersey)
- Strong communication skills
- Experience working with vulnerable populations
