Envision Home Health and Hospice is currently seeking a dynamic Full time LPN to provide exceptional care to patients who require one-to-one attention and monitoring in their home in the Kitsap County area.

Responsibilities include:

• client assessment
• fulfilling patient care plans
• intervention and overall care of patient care
• following physician orders and act in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirements and our policies and procedures.
• strong commitment to delivering exceptional care while demonstrating compassion and integrity

Minimum qualifications

• Graduate of an accredited or state approved school of practical nursing
• Licensure as a LPN in the State of Washington
• A minimum of one year of experience in an acute care setting; home care or hospice experience preferred.
• Phlebotomy and wound care experience preferred.
