What You Will Do as Nurse Practitioner

As a Stonerise Nurse Practitioner (NP), you are a critical member of a patient’s skilled care team at our transitional and skilled nursing care centers (also called “nursing homes”), where we serve both short-term and long-term patients. You serve as an extension of our local Stonerise Medical Director.

As a Stonerise Nurse Practitioner (NP), some of your primary responsibilities include:

• Rounding on acute patients
• Performing staff, patient, and family clinical education as needed
• Rounding on new admissions
• Proactive management of chronic patient conditions

Full-time, Non-Exempt

Education, Licensure Certification Requirements

• Master’s degree from an accredited Nurse Practitioner Program
• Nationally board certified from ANCC or AANP
• Current State of West Virginia Advanced Practice Registered Nurse License
• West Virginia Prescriptive Authority certified or eligible
• DEA Registered or eligible
• Eligible for credentialing requirements
• Prior experience as an NP/RN in post-acute, long-term skilled nursing or similar setting preferred
