Description


The Performance Improvement Coordinator is responsible for detailed analysis of medical record to confirm services are needed for patients enrolled in Quality Based Incentive programs. The Coordinator is also responsible for serving as a liaison with patients to assist with care gap reminders and or scheduling appointment.

Essential Functions and Responsibilities of the Job

  • Maintain a thorough understanding of Quality Incentive Programs and Registry used to monitor patients enrolled in Quality Incentive Programs.
  • Run report of patients due in Quality Registry.
  • Review patient’s medical records for documentation supporting gaps in care.
  • Call patient to educate and assist with scheduling appointments.
  • Ability to be at work and be on time
  • Ability to follow company policies, procedures and directives
  • Ability to interact in a positive and constructive manner
  • Ability to prioritize and multitask

Essential Job Outcomes

  • Maintains current with overview of Quality Metrics programs MCMG is participating in.
  • Maintains responsibility and current knowledge for scheduling appointments throughout the organization for patients enrolled in Quality Metrics programs.
  • Schedules appointments according to individual provider attributes and department guidelines.
  • Performs detailed analysis of medical record to confirm services are needed for patients registered in incentive programs.
  • Document clearly and concisely all incoming and outgoing calls per approved procedure, clearly communicating patient’s needs. Utilize appropriate follow through in documenting messages.
  • Confirm all appointments as directed, print & mail appointment reminders.
  • Establishes and maintains detailed documentation for population.
  • Maintain knowledge of computer scheduling functions, using proper registration, editing existing patient data, and utilizing proper visit types.
  • Work as a team player and communicates in a positive manner with co-workers, managers, and other contacts

Qualifications


Experience

  • A minimum 1 year customer service experience preferably in a medical practice environment.
  • Experience working with electronic medical records and with patients in a physician practice preferred.
  • Must demonstrate excellent organization skills, and attention to detail.
  • Bilingual preffered -Spanish/English

Education

  • Bachelor’s Degree, preferred, High School diploma, GED, or Business Related Course, required
