This Position is for CA Licensed Physical Therapist (PT)

and

Physical Therapist Assistant (PTA)

We are located in the heart of Silicon Valley serving patients in a home health setting all over the Bay Area and Central Valley. Our team is committed to support our field staff with creative ways to overcome the challenges faced by clinicians in community health .

Job Duties Include:

-Monitors and directs patient care to ensure that treatment plan.

-Educate the family and patient in safe and effective interventions and disease process.

-Provides care according to physician’s orders and standard nursing practice.

-Supervise and documents supervision of aides by ensuring that the aide providing care as directed that that documentation of care matches the written plan of care

Pay: $100.00 - $140.00 per hour
