Summary

The Department of Public Health has an opening for a Public Health Nurse (PHN) to join our Nurse-Family Partnership (NFP) team at Downtown Public Health Center to meet the current needs of our community. Nurse-Family Partnership is a national evidence-based community health program in which nurses build therapeutic partnerships with families as they navigate pregnancy and early parenting of their first child. Foundational to the NFP model is that clients are the experts in their lives. Components of the program include a caseload of 25 families per PHN, home visits with families from pregnancy until their child turns 2 years old, weekly reflective supervision sessions with the supervisor, and active participation in weekly team meetings. If not currently trained, the selected candidate for this project will participate in a weeklong training on the Nurse-Family Partnership Program in Denver, CO.

Responsibilities include independent fieldwork providing home visits to clients and their families, caseload management, assessment, care planning, documentation, collaboration with the community to help to reduce barriers to access, patient education, using culturally respectful and client-centered practice. This may include services to infants with special health care needs, abused/neglected children, new refugee and immigrant families and families with complex and multifaceted social concerns.

Job Duties

  • Obtain and document health history and make nursing diagnosis based on nursing assessment.
  • Documentation requires electronic charting: EPIC, and in an NFP database.
  • Provide expertise regarding public health nursing scope, practice and policy at many levels.
  • Provide support services to the client that links/refers, to other Health Department staff and community providers/partners, while assuring access and continuity of care.
  • Provide program planning, monitoring local work plans and progress toward program goals.
  • Participate in quality assurance activities to meet laws, regulations, policies and procedures.
  • Provide training, orientation and education to staff, clients, students and the community.
  • May provide birth control and pregnancy options counseling.

Experience, Qualifications, Knowledge, Skills


Requirements:

  • Graduated from an accredited School of Nursing.
  • Bachelor's degree in nursing science or a graduate level nursing degree of MSN, DNP or PhD
  • Washington State Registered Nurse License in good standing.
  • Proof of current BLS CPR for Healthcare Providers (Attach if current)
  • Washington State Driver’s License and ability to travel within designated territory.

  • Multilingual and/or Multicultural, especially Spanish or Amharic
  • Committed to working with diverse populations with cultural humility.
  • Maternal-child public health nursing experience and lactation training

Work Schedule: Monday through Friday, 8:00 a.m. to 5:00 p.m. This position is subject to the provisions of the Fair Labor Standards Act (FLSA). Please note that this position may include work outside of this schedule based on operational needs.
