Position Summary

The Community Health Navigator acts as a liaison to offer education specific to digital applications and community resources available that may improve a client’s wellness journey, in accordance with the Heartland Health Services’ (HHS) mission, strategic goals, federal and state laws and regulations, performance and outcome objectives, and accreditation standards.

Essential Functions

  • Functions as a reliable intermediary between clients, community-based organizations, and healthcare providers to reduce socio-economic and cultural barriers between clients and institutions.
  • Establishes collaborative relationships with clients using a variety of mediums (in-person, telephone, video call, text, IM, etc.) to assists clients in accessing OSF digital health-related services, targeted kits/supplies, and understanding care guidelines and instructions, as well as helping them navigate the healthcare system while recognizing the model of care the client needs.
  • Utilizes Telehealth and networking systems to connect clients to providers (PCP, specialist, etc.) in real time during off-site visits and troubleshoot systems/equipment in the field; if the client does not have a PCP, work to establish the client with a provider.
  • Maintains all required records, documents, reports, and files as required while strictly adhering to policies and procedures in place.
  • Participates in outreach events and maintains awareness of resources in the community to address client questions about them.
  • Participates in committees, workgroups, and meetings to assist in the development of HHS and Community programs.
  • Functions as a reliable intermediary between provider, client, HHS, and community on a daily, weekly, or as-needed basis according to client needs.
  • Conducts initial consultations and evaluations of individuals to develop health care strategies and connectivity into community resources that are tailored specifically to the client’s need.
  • Provides care coordination services for clients, including the coordination of assessment, treatment information with client’s primary care, behavioral care, specialty care, and/or psychiatry providers as well as with families, referral sources, schools, and/or other professionals, with periodic guidance and oversight of client; documenting all activities in the client record.
  • Participates in program development activities to improve services to clients and community, including making regular recommendations regarding the improvement of services with particular emphasis on audit studies and Quality Improvement. May include helping develop training protocols for new staff and/or participating in professional audits.
Requirements:

Required

  • High School diploma or equivalent
  • Valid Driver’s license and own personal, private vehicle
  • Excellent interpersonal and communication skills, including the ability to integrate constructive feedback
  • Demonstrated ability to communicate effectively via telephone and digitally (video calls, e-mails, texting, instant messaging, etc.)
  • Community Health Worker (CHW) certification required or obtained within 6 months of employment
  • CPR certification required or obtained within 3 months of employment

Preferred

  • Multilingual/bilingual
  • Basic competency with MS Office
  • Experience with or knowledge of healthcare systems
  • Experience with or knowledge of Community Based Organizations (CBOs)
  • Networking and outreach skills and understanding of medical terminology commensurate with those of Community Health Workers and Digital Health Workers
  • Prior experience an Electronic Health Record system
  • Proficient in use of digital diagnostic equipment, digital/electronic platforms, and ability to use computers and/or tablets using multiple applications and Electronic Health Records
  • Experience with evidence-based interviewing skills to create a collaborative relationship with clients
  • Medical Assisting/Certified Medical Assisting background
