Position Summary: Assists individuals who reside in group home residences by providing them with the minimal support necessary to insure the greatest degree of personal choice and independence as needed in daily living and community integration according to goals in their individual support plans.

Essential Job Functions

1-Provides training to assist residents with meeting their individual support plan goals, which may include various aspects of daily living and community integration. Daily living includes meal preparation, housekeeping, laundry, money management, social skills, health and safety, and hygiene and grooming. Community integration is accompanying resident into the community, leading to a more inclusive lifestyle.

2-Possesses and displays knowledge and ability to implement the strengths/needs, interests, goals, communication techniques, behavior plans/interventions, risk plans, etc. of each resident in the home.

3-Provides accurate and timely documentation in areas to include progress towards goals, record of billable services provided, medical issues, behaviors and significant events.

4-Provides self-care assistance to residents including (but not limited to) toileting, bathing, feeding and personal hygiene.

5-Supervises the finances of residents to include assisting in purchasing personal items as approved, maintaining receipts and documentation.

6-Transports residents to meet their needs in agency vehicle (if approved as driver).

7-Role model positive behaviors as you interact with residents, employees and persons in the community.

8-Follows the Health Care Services Monitoring System, which includes documenting and preventing falls, injuries and illnesses.

9-Reports emergencies or incidents which put residents at risk (including suspected abuse or neglect, and injuries of unknown origin) to on-call personnel, following agency policy.

10-Ensures residents have a safe environment.
-Free from abuse, neglect, exploitation and violation of rights.
-Residents participate in fire and tornado drills.
-Communicate medical needs to health services nurse or “on-call” person.

11-Administer medications as prescribed to residents and accurately document.

12-Participate in staff development, inservice training and staff meetings, as required.

13-Complies with agency Policies which include Employee Handbook and Code of Business
Practices and Ethics.

The following physical and environmental factors will vary widely depending on the group home in which hours are being covered and with the residents being served.

Relationships:
-Contact with the public during community activities and resident’s family visiting the group home.

Demands

- Works varying shifts and hours (days, evenings, overnights, weekends, and holidays)

Qualifications: (The following statements represent the minimum experience and training standards which will be used to screen applicants, provided that equivalent substitutions will be permitted in case of deficiencies in either experience or education).

-Prefer experience with MR/DD population.
-Minimum 18 years of age.
-Ability to read and write full sentences and comprehends written instructions.
-Ability to deal with behavior implications of residents, possesses patience and understands the individual’s ability to progress.
-Ability to effectively communicate with residents, staff and the public.
-Ability to apply basic math including addition, subtraction, multiplication, division and calculation of money.
-Ability to successfully pass training and remain certified in the following within 90 days of employment: First Aid/CPR, Medication Administration and Positive Intervention Techniques.

License:
- Maintains a valid driver’s license (including three years of driving experience) with acceptable driving record.
