Rehabilitation Coordinator Position Description

Job Requirements:

A. Education at a minimum: a. Associate's degree/Bachelor's degree in a health-related field or 30 hours, or their equivalent, of college credit towards a bachelor's degree in a health-related field. b. 1 year of work experience in a supervised mental health setting where the employee is responsible for implementing the rehabilitation activities outlined in the client's IRP. c. Full background check: Potential staff members must pass a full state and federal criminal background check. d. 60-hour orientation: Staff members must have 60 hours of on-the-job direct PRP supervision before providing services to minors served by the program without direct supervision. e. Transportation: Staff members must possess a valid driver's license and maintain and clean driving record. The position requires the use of a personal vehicle. This is a mobile position.

Duties and Responsibilities:

A. Staff members will be working one-on-one with clients and families
B. Management of symptoms (when applicable)
C. Advocating for families
D. Attend all staff meetings and training
E. Conducting home visits
F. Working alongside treating Therapist and interdisciplinary team to develop goals and objectives
G. Implementing goals and objectives
H. Developing recreational projects for youths and families to foster a healthy reintegration into the community
I. Required to write progress notes (weekly) that summarize of each contact with every client
J. Staff will work on rehabilitation activities such as: a. Age-appropriate self-care skills include personal hygiene, grooming, nutrition, dietary planning, food preparation, and self-administration of medication. b. Social skills include community integration activities, developing natural supports, developing linkages with, and supporting the client's participation in community activities. c. Independent living skills include maintenance of the client's living environment, community awareness, mobility skills, money management, and accessing available entitlements. d. Activities that support the client's cultural interests, conflict resolution, anger management, interactive skills with peers and authority figures, maintaining personal living space, maintaining age-appropriate boundaries, maintaining personal safety in a social environment, and time management including constructive use of structured and unstructured time.

Skills:

A. Ability to provide therapeutic interventions
B. Good communication skills, written and verbal
C. Able to deal constructively with conflict
D. Ability to plan, coordinate, and direct varied and complex operations
E. Ability to travel throughout the community to make home visits, attend meetings and participate in community educational program

Required education:

A. Minimum high school diploma with 4 years experience working with mental and emotional disturbed youth and adults AND OR

B. Bachelors, Master's degree (Human Service, Social Work, Psychology)
